1. Acceptance of Terms
Welcome to Central Park Advanced Imaging ("CPAI," "we," "us," or "our"). By accessing our website, scheduling an appointment, or using any of our imaging services, you ("Patient," "you," or "your") agree to be bound by these Terms and Conditions.
These terms apply to all patients, referring physicians, and visitors to our facility and website. If you are scheduling services on behalf of another person, you represent that you have the authority to agree to these terms on their behalf.
2. Description of Services
CPAI provides advanced diagnostic imaging services including, but not limited to:
- Magnetic Resonance Imaging (MRI)
- Computed Tomography (CT) Scans
- Positron Emission Tomography/Computed Tomography (PET/CT)
- Ultrasound
- Full-Body Preventative Screening
- Specialized diagnostic imaging services
All imaging services are performed by licensed technologists and interpreted by board-certified, subspecialty-trained radiologists. We reserve the right to modify, suspend, or discontinue any service at any time without prior notice.

3. Appointments and Scheduling
3.1 Booking Appointments

3.2 Appointment Confirmation
Once scheduled, you will receive confirmation via email and/or phone. Please verify all appointment details, including date, time, and preparation requirements. It is your responsibility to notify us immediately if any information is incorrect.
3.3 Arrival Time
Please arrive 15 minutes before your scheduled appointment time to complete necessary paperwork and preparation. Late arrival may result in rescheduling your appointment.
3.4 Referrals
Some imaging procedures may require a referral from a licensed physician, depending on insurance requirements. We will verify referral requirements during scheduling.
4. Payment Terms
4.1 Payment Due
Payment is due at the time of service unless prior arrangements have been made. We accept:
- All major credit cards (Visa, MasterCard, American Express, Discover)
- Debit cards
- HSA/FSA cards
- Cash or check
4.2 Self-Pay Pricing
Self-pay prices are listed on our website and are subject to change. Prices include the imaging procedure and radiologist interpretation. You will be provided with a detailed cost estimate before your appointment.
4.3 Payment Plans

4.4 HSA/FSA Eligibility
All of our imaging services are eligible for HSA (Health Savings Account) and FSA (Flexible Spending Account) payment. We will provide all necessary documentation for reimbursement.
5. Insurance and Billing
5.1 Insurance Verification
We accept most major insurance plans. We will verify your insurance coverage prior to your appointment, but coverage verification is not a guarantee of payment. You are responsible for understanding your insurance benefits, including deductibles, co-pays, and out-of-pocket expenses.
5.2 Insurance Claims
We will submit claims to your insurance provider as a courtesy. However, your insurance policy is a contract between you and your insurance company. You are ultimately responsible for all charges, regardless of insurance coverage.
5.3 Out-of-Network Coverage
If we are out-of-network with your insurance provider, you may still receive partial coverage. We will provide you with a detailed invoice (superbill) that you can submit to your insurance company for reimbursement.
5.4 Patient Responsibility
You agree to pay any amount not covered by insurance, including deductibles, co-insurance, and denied claims. Payment is expected within 30 days of receiving a statement.
6. Cancellation and Rescheduling Policy
6.1 Cancellation Notice
We require at least 24 hours notice to cancel or reschedule your appointment. You may cancel or reschedule by:

6.2 Late Cancellation Fee
Cancellations made less than 24 hours before your scheduled appointment may be subject to a cancellation fee of $150. This fee helps cover the cost of reserved time and staffing.
6.3 No-Show Policy
Failure to show up for a scheduled appointment without prior notice will result in a no-show fee of $200. Repeated no-shows may result in our refusal to schedule future appointments.
6.4 Emergency Circumstances
We understand that emergencies happen. Cancellation fees may be waived in cases of documented medical emergencies or other extenuating circumstances at our discretion.
7. Medical Information and Records
7.1 Accuracy of Information
You agree to provide complete and accurate medical information, including medical history, current medications, allergies, and any implanted medical devices. Failure to disclose relevant medical information may compromise the safety and accuracy of your scan.
7.2 Medical Records Access
You have the right to access your medical imaging records in accordance with HIPAA regulations. Requests for records must be made in writing and may be subject to a reasonable processing fee.
7.3 Image Storage
We retain imaging studies and reports according to state and federal regulations. Images are stored securely on HIPAA-compliant servers.
7.4 Report Distribution
Imaging reports will be sent to your referring physician (if applicable) and made available to you through our secure patient portal or by request. Results are typically available within 24-48 hours.
8. Privacy and HIPAA Compliance
CPAI is committed to protecting your privacy and complying with the Health Insurance Portability and Accountability Act (HIPAA). We maintain strict confidentiality of all patient information and medical records.
8.1 Protected Health Information
Your protected health information (PHI) will only be used and disclosed as permitted by HIPAA regulations, including:

- Payment and billing operations
- Healthcare operations
- As required by law
- With your written authorization

9. Patient Consent and Responsibilities
9.1 Informed Consent
By proceeding with imaging services, you acknowledge that:

- You understand the benefits and risks
- You consent to the procedure
9.2 Contrast Administration
Some imaging procedures may require the administration of contrast agents. You will be informed if contrast is needed and will be asked to sign a separate consent form. You must disclose any allergies or kidney problems prior to contrast administration.
9.3 Pregnancy and Breastfeeding
If you are pregnant, think you might be pregnant, or are breastfeeding, you must inform our staff immediately. Some imaging procedures may not be safe during pregnancy.
9.4 Patient Behavior
You agree to conduct yourself in a respectful manner toward our staff and other patients. We reserve the right to refuse service to anyone who is disruptive, abusive, or under the influence of alcohol or drugs.
10. Intellectual Property
All content on the CPAI website, including text, graphics, logos, images, and software, is the property of Central Park Advanced Imaging or its content suppliers and is protected by United States and international copyright laws.
You may not reproduce, distribute, modify, or create derivative works from our website content without express written permission.
11. Limitation of Liability
11.1 Service Disclaimer
While we strive to provide the highest quality imaging services, we cannot guarantee specific outcomes or results. Medical imaging is a diagnostic tool and may not detect all conditions.
11.2 Limitation of Damages
To the fullest extent permitted by law, CPAI shall not be liable for any indirect, incidental, special, consequential, or punitive damages resulting from your use of our services.
11.3 Professional Liability
Our radiologists carry professional liability insurance as required by law. Any claims of medical malpractice must be handled in accordance with New York State law.
Important
Nothing in these terms limits or excludes liability that cannot legally be limited or excluded under applicable law.
12. Modifications to Terms
CPAI reserves the right to modify these Terms and Conditions at any time. Changes will be effective immediately upon posting to our website. Your continued use of our services after changes are posted constitutes acceptance of the modified terms.
We encourage you to review these terms periodically. The date of the last revision will be indicated at the top of this page.
13. Governing Law
These Terms and Conditions shall be governed by and construed in accordance with the laws of the State of New York, without regard to its conflict of law provisions.
Any disputes arising from these terms or your use of our services shall be resolved in the courts of New York County, New York. You consent to the personal jurisdiction of such courts.
13.1 Severability
If any provision of these terms is found to be unenforceable or invalid, that provision shall be limited or eliminated to the minimum extent necessary, and the remaining provisions shall remain in full force and effect.
